Coaching Requirements
ALL coaches must complete the following BEFORE the start of the season.
A coach with these credentials must be present with the team at ALL times.

All Volunteer Coaches
Please bring the below two forms along with you when you come in to get fingerprinted, make sure the applicant fills out name (print & sign), date, phone number, and signs the bottom of the waiver and fills in their name and date on the billing sheet.
**Please have each one of your volunteers bring the completed waiver, billing sheet and driver’s license/picture ID with them for their appointment.**
HOURS OF OPERATION:
Monday through Friday from 8:30 a.m. to 4 p.m. Appointments are necessary at this time, same day appointments are always available. Please have your volunteers call to schedule an appointment. We cannot fingerprint anyone that is ill.

• BCI background check; renew every 5years
Lived Outside Ohio during the last 5 years
• BCI/FBI background check; renew every 5 years
